HRD Effects on First-line Adjuvant Chemotherapy and PARPi Maintenance Therapy in Chinese Ovarian Cancer Patients

This study is the largest to date examining the homologous recombination defect (HRD) status in epithelial ovarian cancer (EOC) among the Chinese population, including 463 patients. Through the analysis of first-line adjuvant chemotherapy (FACT) and first-line PARP inhibitor (PARPi) maintenance therapy (FPMT), the study utilizes the established HRD score methodology to accurately assess the HRD status of tumors. A novel combination of niraparib and anlotinib in platinumresistant ovarian cancer: Effificacy and safety results from the phase II, multi-center ANNIE study

This study conducted gene testing and data analysis on 40 patients with advanced platinum-resistant recurrent ovarian cancer who received the combination therapy of niraparib and anlotinib. The results indicate that the "Anni combination" exhibits favorable anti-tumor activity and tolerable toxicity in patients with platinum-resistant recurrent ovarian cancer who have undergone multiple lines of treatment. Additionally, through exploratory gene analysis of 56 plasma samples (baseline and follow-up) from 12 patients, it was observed that PPM1D mutations may lead to resistance to the "Anni combination," while SPEN mutations may be associated with better response to this combination therapy. In this study, 40 ovarian cancer patients' patient-derived xenografts (PDX) were subjected to sensitivity testing with niraparib, olaparib using whole exome sequencing (WES) and HRD scoring. The results aligned with clinical experience, confirming the utility of PDX as an in vitro model that accurately reflects the therapeutic efficacy of PARP inhibitors (PARPis). We collected fresh cancer tissues from a consecutive cohort of 158 patients to construct subcutaneous PDX models. Paired samples of original tumors and PDX tumors were compared at the genome, transcriptome, protein levels, and the platinum-based chemotherapy response. we found to retain the histomorphology, protein expression, and genetic alteration patterns of the original tumors,which demonstrated a high degree of similarity with patients in terms of the chemotherapy response and could predict prognosis. We identified somatic driver candidates, including significantly mutated genes KRAS and KIT and copy-number alteration drivers, including deleted ARID1A and PARK2, and amplified ZNF217, CDKN1B, and KRAS. YSTs had very rare TP53 mutations, whereas the tumors from patients with abnormal gonadal development contained both KRAS and TP53 mutations.
